Collaboration and Leadership Reflection

Hello everyone, thank you for joining me today. On today’s reflection, I want to reflect on our interprofessional collaboration experience within palliative care within the hospital setting. I will be drawing from insights by the interdisciplinary team members, including Dr. Richardson, Nurse Mitchell, and other professionals with whom we collaborated with in the palliative care. Notably, the case study provides a glimpse of the complexities and challenges encountered among interdisciplinary teams providing palliative care. The reflections, as shared by members of the team showcase the strengths and areas in need of improvement within the team’s collaborative practices. One of the aspects of reflection is the emphasis on the importance of effective communication. In this reflection, Dr. Richardson accounted for miscommunication leading to delayed pain management and underscores the need for clear and timely communication among team members. In the palliative care, she highlighted the essence of communication within the team. Besides, she mentioned a situation where lack of communication caused delayed adjustments in pain management for a patient (Pornrattanakavee et al., 2022). As a result, this led to unnecessary suffering. At the same time, Dr. Richardson emphasized on the need for involved people of different disciplines and collaborative skills in challenging family discussions to yield support for patients and their families.  Evidence-Based Communication Strategies to Impact Patient Essay

Next, from the interdisciplinary collaboration, it was clear that emotional burnout is prevalent among professionals working closely with patients in end-of-life situations. It shows the importance of self-care and support systems within a team to maneuver the emotional toll. In the case study, Nurse Mitchell raised concerns on emotional burnout in the team citing the loss of a patient that deeply affected her. She also mentioned instances of coordination challenges including a mix-up of medication that underscores the need to improve care coordination protocols and thus prevent further errors.

Further, Mr. Hayes, a part of the interdisciplinary team, described certain complexities when providing patient care. Notably, Mr. Hayes was a lead social worker in the interdisciplinary team. In his capacity, he shared a situation where cultural and family dynamics played a profound impact in influencing patient care. Such situations challenged the process of provision of medical care. He emphasized the need for involvement of other department heads involved in the management of complex familial situations to improve patient care. The insights highlight the need for enhancing cultural competence and sensitivity among the team to ensure that care is tailored to an individual’s needs and preferences (Myrhøj et al., 2023). From the case study, one can see the importance of continued training and education across the team, which echoes the importance of continuous learning and professional growth. It highlights the value of interdisciplinary understanding and collaboration within a holistic patient care setting. At the same time, effective documentation and clearly defined roles for volunteers is integral (Thiel et al., 2020). The reflection provided by Mrs. Martinez in the case study regarding her overall experience of interdisciplinary collaboration in patient care states the importance of documentation and clearly defined responsibilities for volunteers. The reflection shows the need to consider ways that would help in improving communication channels and support structures to enhance contribution of the team. Conclusively, from this reflection, it remains clear that the interdisciplinary team was dedicated and compassionate in caring for patients with life-long illnesses. There are certain opportunities for growth in interprofessional collaboration. Besides, for it to be effective, there is a need to address communication barriers and provide support to each other emotionally hence enhancing cultural competence. Continuous learning and clarity of roles and responsibilities would help in improving the quality of care while providing the best possible outcomes for patients in palliative care settings and their families.  Evidence-Based Communication Strategies to Impact Patient Essay
