Assessment details Instructions
Assessment overview The objective of this assessment is to assess your knowledge and performance in performing preliminary and ongoing physical health assessments of all body systems, gathering data that contributes to a person’s individualised health care plan.

This unit applies to enrolled nursing work carried out in consultation and collaboration with registered nurses, and under supervisory arrangements aligned to the Nursing and Midwifery Board of Australia regulatory authority legislative requirements.

The skills in this unit must be applied in accordance with Commonwealth and State/Territory legislation, Australian/New Zealand standards and industry codes of practice.

Instructions for this  assessment This is a project-based assessment that assesses your knowledge and performance of the unit.

This assessment is in 1 part:

1.      Research based.

·         Assessment feedback.

Submission instructions On completion of this assessment, you are required to submit it to your Teacher/Assessor for marking. Where possible, submission and upload of all required assessment files should be via the TAFE NSW online learning platform.

It is important that you keep a copy of all electronic and hardcopy assessments submitted to TAFE and complete the assessment declaration when submitting the assessment.

What do I need to do to achieve a satisfactory result? To achieve a satisfactory result for this assessment you must answer all the questions correctly.

If a resit is required to achieve a satisfactory result it will be conducted at an agreed time after a suitable revision period.

What do I need to provide? •        TAFE NSW student account username and password. If you do not know your username and password, contact your campus or service centre on 131601.

•        Computer or other device with word processing software and internet access.

•        Writing materials, if required.

What the Teacher/Assessor will provide Access to this assessment and learning resources, including the student workbook and any supporting documents or links.

Computers, data sheets, reference text, organisational policy etc that is referenced in the assessment. These may be hard copy or made available online.

Assessment feedback, review or appeals In accordance with the TAFE NSW policy Manage Assessment Appeals, all students have the right to appeal an assessment decision in relation to how the assessment was conducted and the outcome of the assessment. Appeals must be lodged within 14 working days of the formal notification of the result of the assessment.

If you would like to request a review of your results or if you have any concerns about your results, contact your Teacher/Assessor or Head Teacher. If they are unavailable, contact the Student Administration Officer.

Part 1: Research based

For this assessment, you are required to complete 7 briefs and a bibliography.

Conduct research from the provided textbook and resources; and from research articles from library databases (optional) and document your research findings.

A word limit for each brief is indicated to provide you with guidance for the size of your research responses.

At the end of your project include a bibliography using APA reference style.(For guidelines use the following link: A guide to TAFE NSW referencing: APA 7 style)

Your responses must be submitted on a word document with each response clearly identified by the number, for example:

Once completed you will need to submit this assessment to your Teacher/Assessor for marking.

Complete a growth and development table based on the age and gender group stated for each person listed below.

You may use the Brief 1 table template in the appendix or create your own.

Your answer should be a minimum of 75 words but no longer than 100 words for each person’s growth and development table.

Define the six components of health, including the physiological and psychosocial aspects of health.

Complete the developmental stages, health risks and major activities for the listed people in adolescence and adulthood.

Your answer for each person should be a minimum of 30 words but no longer than 50 words.

Brief 7

Mr William Smith, 81 years old, was admitted to the ward with confusion and agitation. After you commenced the shift at 0700 hours, you noticed that Mr Smith was not responding to your verbal command, only to pain stimuli.

  1. How will you recognise if Mr Smith is deteriorating?
  2. How would you determine Mr Smith’s level of consciousness? Is there any particular tool that you might use to check his level of consciousness?
  3. How and when will you raise your concern for Mr Smith?
